Dr Jacobs is based at the Royal Free London NHS Foundation Trust. He provides clinical negligence reports covering all aspects of acute medicine and general medicine including preliminary and full breach of duty, causation, condition & prognosis and life expectancy reports.
He attends conferences with counsel, provides joint expert statements, and has given evidence as an expert in coroner’s court.
He has nine years of medico-legal experience and has provided around 300+ expert opinions.
Dr Jacobs is an independent medical examiner at the Royal Free London. The role involves the scrutiny of medical records to establish cause of death, liaising with the clinical team involved with the care of the deceased, and the deceased’s family.
Coroner referrals are made when necessary and clinical governance/organisational learning issues are also highlighted as and when appropriate.
